The present study aimed to design an action plan to improve factors associated with adherence to prenatal care among pregnant adolescents attended at the Provincial Hospital Dr. Francisco Antonio Gonzalvo. The research was framed within a quantitative approach, descriptive, observational, and cross-sectional in nature, with prospective data collection, using a non-experimental design that allowed the analysis of clinical records from fifty pregnant women, constituting a census sample. The sociodemographic results show that most of the pregnant women are between fourteen and sixteen years old, with a predominance of secondary education and urban origin, while the most frequent marital status is single. Regarding reproductive history, the vast majority are experiencing their first pregnancy, with interpregnancy intervals not applicable in most cases, and minimal previous obstetric history. With respect to prenatal care, a considerable number initiated follow-up during the first trimester, and the observed adherence was mainly moderate, followed by high and low. These findings allow for the characterization of the pregnant population and the factors associated with attendance at prenatal visits. The information obtained supports the development of an action plan aimed at improving adherence through strategies that consider personal, obstetric, and institutional factors. Expert validation confirmed the relevance and applicability of the plan, with high ratings in all evaluated aspects, including clarity, timeliness, organization, coherence, methodology, and relevance, reflecting the consistency and robustness of the instrument to guide effective interventions in the pregnant population.
